An employee was helping a customer place a trailer hitch onto the hitch ball. While determining why the hitch would not go onto the ball, the employee placed his index finger up into the hitch. At the same time, the customer placed his vehicle in park and took his foot off of the brake. The vehicle moved and pinched the employee's index fingertip, resulting in amputation.

An employee was helping a crew erect scaffolding. As he applied pressure to a component, it gave way and causing the employee to fall from the scaffold to the ground. The employee sustained a dislocated elbow that required surgery.

On August 31, 2023, an employee placed a hydraulic controlled attachment on a skid-steer. The attachment fell off and contacted the employee's left leg, resulting in an injury.

Employees were loading pipes onto a flatbed truck when one of the pipes rolled off the truck and struck the injured employee, causing a fractured vertebra.

On December 21, 2022, an employee was loading and unloading a flatbed truck. The employee jumped from the ground to get onto the trailer but did not clear the trailer and his knee struck it. The employee sustained a laceration to his knee.

An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was helping to install a tarp on a roof when they fell through a skylight to the ground below, resulting in fractures to their face and ribs.

On August 5, 2025, an employee was exiting a room when his left middle and ring fingers became caught in the door as it shut. The middle fingertip was amputated.

An employee was driving a forklift and unloading racks of glass from a truck. A rack began to tilt; the employee exited the forklift and went to stabilize it. The rack tipped and pinned his left arm against the forklift. The forearm was broken and required surgery.

An employee was sitting on a stack of steel beams. When they stood up, the top beam fell onto the employee, resulting in a broken left leg.

An employee was working to tow a forklift with another forklift when the tow line came loose and struck their face. The employee sustained head, face, and eye injuries.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
