A sanitation employee was cleaning a dough cruncher/roller machine when two fingers on his right hand were amputated between the nail and knuckle. At the time of the incident, the blade was not in the machine, but the machine was running.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was attempting to straighten a wooden pallet with a bar. The pallet was positioned on top of a frame conveyor system 1 foot off of the ground. While maneuvering the bar, the employee lost balance and fell backward onto the concrete floor, extending their right hand to catch themselves as they fell. The employee suffered two fractured bones in their wrist and one fractured bone in their right hand.

An employee was working near a conveyor when his left middle finger was caught in the conveyor. The employee required surgery and their finger was partially amputated.

An employee was filling a hopper that deposited blueberries in muffin trays. Two plastic plates inside the equipment closed, pinching and amputating the employee's left ring fingertip. The equipment was running and unguarded at the time.

An employee had been using soap and a towel to clean a slicer. They were finishing the cleaning procedure when the power switch was activated. The slicer turned on while the employee's right ring finger was in the blade gap and the blade amputated their fingertip without bone loss.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
