An employee was unloading ductwork from a truck to a skip pan. The employee slipped and fell through a 2- to 3-foot gap between the truck and the pan, landing on the ground 3 feet 8 inches below. The employee hit the side of the truck while falling, suffering five broken right-side ribs (one of which punctured the lung).

An employee went to the roof to repair a belt on a fan. The fan was turned off but was not completely done rotating at the time. The employee was trying to manually slow the fan when the moving belt pulled his hand inside the belt and rotated it through the pulley system, where it pinched off his left index fingertip.

RJ Tide Construction Company, INC.

An employee was spraying concrete sealer when they fell into a 3-foot drain hole. The employee landed on their feet and sustained a broken femur.

Luminous Personnel

A temporary employee walked across a wooden pallet to retrieve product to stock. When he walked back across the pallet, it broke, causing the employee to fall to the ground and sustain a fractured hip.

A & K Rentals, LLC

On April 10, 2023, at 12:00 PM, an employee was using a broom to clear rock from a building's foundation in preparation for a spray-on vapor barrier. There was a 4-foot deep trench next to the building where the employee was working and the soil at the trench's edge fell away. The employee fell into the trench and was hospitalized for a left pelvis and/or femur fracture.

C&S Enterprise, Inc.

An employee was walking up a steel staircase to check wiring in the electrical room. The staircase had a top platform with a grate that was sitting across the beams of the platform. The grate had shifted and as the employee stepped on it, it gave way, causing him to fall approximately 3 feet to the ground below. The grate landed on the employee and he was hospitalized for four transverse process fractures in the lower spine.

Sams Club

An employee was stocking the freezer and stepped on a wood pallet. The employee's foot went into the pallet and they sustained a right ankle fracture that required hospitalization and surgery.

Channel Sheet Metal, Inc.

An employee was driving a forklift when its right front wheel dropped off a 6-inch curb. The forklift rolled over and trapped him against the concrete surface. He suffered a broken pelvis, ribs, and collarbone, as well as a collapsed lung.

Blackburn's Fabrication, Inc.

An employee was operating a brake press when the top die broke, causing the bottom die to roll out and onto the employee's legs. Both of the employee's legs were crushed below the knee, and both legs were partially amputated. The employee's left leg was also fractured above the knee.

An employee was performing routine maintenance on a shear machine when the machine's foot pedal was engaged and the blade contacted his right middle finger, resulting in a partial amputation.

An employee was helping to tighten down a metal packaging band for shipping. The metal band broke and struck the employee across their face, cutting their left face/eye area. The employee was hospitalized.

An employee was cutting plywood on a table saw. They went to retrieve a piece of wood and the saw blade partially amputated a fingertip.

GS II Building Products, Inc.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

Bull Moose Tube

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
