Wire began to spool at the top of a draw machine. The injured employee used pliers to move the wire to the bottom of the draw machine. The pliers became caught in the wire and then the employee's glove was caught in the wire, resulting in the amputation of their right index finger at the first knuckle.

An employee was checking the tension of a belt and pulley on a drawing machine when their left hand was pulled into the machine and their fingers were caught in the belt. The employee's index and middle fingers were amputated to the first joint, their ring finger was amputated to the nail bed, and their little finger was fractured.

An employee was setting up a pole and a floor jack to reposition wire inside of the drawing machine. When the machine was turned on, the wire struck the pole, causing the floor jack to kick out. The employee was struck by the jack and sustained a left kidney laceration that required hospitalization.

An employee had been running a weave machine which makes chain link. The employee made an adjustment by tightening a chain to a gear and then went to lunch. Upon returning from lunch, he turned on the machine and noticed the guard on the back of the machine was on the floor. The employee picked up the guard to put it back in place. The gear pulled in his gloved left hand and amputated the pads of the two middle fingers.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
