An employee was standing on a pallet that a forklift operator had lifted approximately 10 feet in the air to remove an exhaust pipe. The employee fell from the pallet while attempting to remove the exhaust pipe
and struck his left wrist on a water pipe as he fell to the floor. He suffered a severe fracture of his left wrist resulting in hospitalization.

An employee was using straps to secure a load inside a transport trailer. The employee was standing on a stack of finished goods that was three pallets high. Each pallet was 16 inches high. He was walking from the stack of three pallets onto a stack of two pallets when he lost his balance and fell onto another stack of finished goods (that was two pallets high) and then landed on the floor of the trailer. The employee was hospitalized with fractured ribs.
An employee was using a utility knife to cut a roll of scrap plastic when the knife cut the end of a finger on his left hand resulting in a partial fingertip amputation.
