On Sunday June 7, 2015, at approximately 4:55 p.m., an employee on the production floor was operating a crimper machine, which is a wire connector crimping machine operated by an unguarded foot pedal. The injured employee was hand-feeding wire pieces into the point of operation when she inadvertently pressed a foot pedal with her right-hand index finger, resulting in an amputation. The point of operation was unguarded at the time.
