An employee tripped over a pallet jack and fell to the ground, suffering a broken hip.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was descending the ladder of an elevated cargo loader when he slipped and fell, landing on the ground about 10 feet below. He suffered head and back injuries including fractured vertebrae, an acute subdural hematoma, and an intraparenchymal hematoma of the brain.
An installation team unpacked a crate of actuator arms and placed them on blocks. One of the blocks shifted, causing an actuator to fall. When an employee attempted to catch the actuator, it crushed three fingers on his left hand.
