Final narrative
The injured employee climbed up a ladder to access the top of a concrete ledge, which was part of the support structure for a furnace, and was standing on the north side of a cylinder as another employee was operating a lift to align the pin on top of the cylinder. A third employee was working a come-a-long to help align the cylinder top. The injured employee was flagging from the platform when he fell approximately 8 feet, landing on the concrete surface below. He was hospitalized with a fractured tibia and fibula in the left leg.
An employee was using a sleever bar to move a checker plate into place and secure it to the floor of a structure in the melt shop. The sleever bar became stuck and the employee used his left hand to lift the checker plate up enough to free the sleever bar. Once the bar was freed, the checker plate came down and pinched his left index finger between two pieces of plate. The employee suffered an amputation of soft tissue at the fingertip, which required hospitalization.
On May 16, 2025, an employee crossed over a stand to support a wide flange beam they were fabricating. The beam rolled over onto their lower right leg and foot, resulting in fractures.

The crane rail was being pushed on rollers toward the employee's location and as he tried to pry the end of the crane rail up to get it on top of the next roller, he lost his balance and fell 36 feet to the ground below. The employee sustained fractures to their right ankle, pelvis, and left arm. The employee was wearing a personal fall arrest system, but was not tied off at the time.
An employee was installing drive shafts on an overhead crane. The drive shaft was rigged about 17 feet up with a wire rope choker. The drive shaft began slipping through the rigging, when it slid down and struck the injured employee's hard hat. He was knocked to his knees and landed on his right knee. The employee sustained an amputation to the right leg below the knee.
An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

On October 21, 2025, an employee was operating a saw when a pneumatic clamp caught their right middle finger. The employee suffered a partial amputation to the finger.
An employee completed an end cut on a metal shim and started transferring the metal shim from the saw conveyor to two saw horses in the area. The employee set one end of the shim (7 feet long x 8 inches wide, weighing 218 pounds) on one of the saw horses, then slid the other end onto the other saw horse and the shim fell from the first saw horse. The employee's left index fingertip was pinched between the saw horse and the shim, resulting in a skin avulsion. The fingertip was amputated later.
An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
