On September 25, 2025, an employee was exiting a yard truck. They slipped and fell approximately 3 feet from the top tread of the steps to the concrete below. The employee sustained a fractured left hip.

An employee was moving a cardboard tote of product using an electric pallet jack. After he turned a corner, he came in contact with a round metal bracket that holds a trash bag mounted to a pole. The pallet jack continued forward and his right hand got caught between the pole and the handle of the jack, resulting in an open fracture and crush injury to his thumb that required hospitalization and surgery.

An employee was removing product from a leg quarter saw with a hook when the hook became jammed. As he was getting the hook free, his right hand came into contact with the saw, which partially amputated his middle finger.

An employee was standing at a workstation, preparing to hang poultry. The employee reached to close a manual air damper for an air vent, which caught their left middle finger as it closed. The employee suffered an amputation to the fingertip.

An employee was waiting for test product to come out of a freezer. While the employee was lowering the basket used to catch the test product, their left little finger was caught between the basket and the side shield on the freezer slide chute, resulting in an amputation to the fingertip.

On August 22, 2022, an employee began experiencing neck and right shoulder pain during her shift after performing job-related tasks such as repetitive lifting. She was hospitalized for a right shoulder sprain and a pinched nerve.

An employee was working in the back room putting boxes on carts to stock the store. He suffered a heart attack.

An employee was manually lifting cases of beer from a rolling cart and stacking them in a storage room. The employee felt ill, experienced chest pain, and then passed out and struck his head.

After pulling a 36-pound skid into place and then repeatedly moving 14-pound boxes, an employee suffered a muscle tear in the lower right abdomen. She was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.

An employee was troubleshooting a thigh deboner on a poultry processing line. As he worked to dislodge a thigh bone caught in the blade mechanism, the machine amputated the first and second digits on his left hand. He was hospitalized.

On September 17, 2025, an employee was cleaning a cutting machine when the cutting blade amputated their right index finger below the first knuckle.

An employee was operating a stand-up forklift, picking product for an order. The forklift struck a metal rack and the employee's left arm was caught between the forklift and the rack. He was hospitalized with a fracture to the neck of his radius and an open fracture to the olecranon (elbow).

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
