The employee's right index and middle fingertips were amputated at the first knuckle.

The blade snagged on the sprue and pulled his left index finger into the blade, which cut it. The finger was partially medically amputated.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.
