An employee was helping to drive pilings when their finger became pinched between a pile and the pile lead resulting in a partial amputation.

An employee was manually unloading sections of counterweight pipe. Each pipe had swiveling D-rings mounted on the ends to be used for lifting the pipes with slings. The employee's finger got caught between a D-ring and a pipe resulting in a partial finger amputation.

An employee was welding from a bucket truck when sparks dropped into the bucket and caused a fire. The employee sustained burns to the legs and back.

An employee was attempting to remove a rock that had jammed a mechanical gate on a batch plant hopper and conveyor. When the rock became free, the gate snapped shut, pinching the employee's hand and fingers between the gate and the stop point of the gate. Four fingers were amputated at the second joint of the knuckles.

An employee was painting a 4" pipe when he tried to clean the residual paint off the paint mixer drill bit attachment. He was using a nylon sack to wipe the excess paint off when the drill was inadvertently activated. His left pinky finger became wrapped in the nylon sack material, resulting in a finger amputation.

Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.

An employee was doing a daily inspection of the sifter screen on a mixer when the mixer line activated. The armature for the screen struck the employee's right wrist. Their wrist was cut and their hand was fractured.

An employee was being trained on a machine when a roller came down on their hand, resulting in a fracture and laceration.

An employee was doing a dry run during the setup of an ice cream machine. The machine indexed and the filler head amputated the employee's left middle finger above the last knuckle.

An employee was on a 3-foot ladder guiding cable onto a spool during installation on an overhead door. As the control box was jogged, the employee's hand was caught between the cable and spool, resulting in a wrist and hand fracture.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee fell while stepping off the tailgate of a pickup truck. The employee landed on the ground and suffered a broken left fibula and tibia.

On October 7, 2025, employees were conducting trenching operations for the installation of a new sewer line system. The excavator's bucket was detached and put inside the trench as it was being exchanged for a different-sized bucket. An employee entered the trench with a shovel to move some dirt located at the front end of the pipe. The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.

Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.

An employee was lifting a piece of 30-inch line stopping equipment using a jack screw. A lifting eye weld snapped and the load dropped. The end of the jack screw then landed on the employee's heel and crushed it. The employee was hospitalized.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
