An employee fell while walking down a stairway, landing on the concrete floor at the bottom and suffering a head laceration. The employee was hospitalized.
An employee was clamping a cast iron gas meter housing unit into a hydrostatic test unit. While actuating the clamping mechanism, he was holding the body with his right hand. As the clamping mechanism was clamping, the cap on the outlet end (left side of the body) shifted. At this point, he grabbed the body and cap to prevent it from falling out of the testing machine. His left thumb then became caught between the cap and the hydrostatic test clamping mechanism, causing a partial amputation of his thumb tip.
