An employee was driving a forklift when it struck a structural steel column, which separated the mast from the forklift. The right side of the employee's skull struck a front support post in the forklift cabin, and the employee suffered a neck fracture and a concussion.

An employee was moving a tool box when its top drawer started to open. While the employee was stabilizing the tool box and drawers, their left middle and ring fingertips were amputated through the nailbeds. The fingertips were surgically reattached.
