A maintenance employee was fixing a refrigerator diffuser in a produce cooler that was leaking water. The produce cooler has dock doors for truck deliveries. The employee was squeegeeing the water out of the produce cooler when he slipped on the water and fell off the dock onto the concrete below. He suffered a broken hip.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
