Final narrative
An employee was on a barge, pulling rigging on top of a stack of steel pipe pilings. He fell off the end of the stack, landed on crane mats used as dunnage, then fell 7 feet to the deck of the barge. His right leg struck a 6-by-6 piece of dunnage lying on the deck, and he suffered a broken femur.
On 11/22/2023, at 8:35 AM, an employee was stepping onto a barge when they slipped on a wet surface and fell on their knee. The employee sustained a torn ligament in their knee requiring hospitalization and surgery.
After moving a barge lid, an employee was walking down the stairs on the barge when they slipped and fell approximately 15 to 20 feet, reuslting in a head injury.
An employee was using an excavator on a barge to scoop and remove fertilizer product stored in the cargo area. As the employee was transporting the skid-steer by chains and hoisting equipment from the dock to the barge, the employee fell approximately 10 feet off the skid-steer bucket to the cargo hold area below. An employee completed an end cut on a metal shim and started transferring the metal shim from the saw conveyor to two saw horses in the area. The employee set one end of the shim (7 feet long x 8 inches wide, weighing 218 pounds) on one of the saw horses, then slid the other end onto the other saw horse and the shim fell from the first saw horse. The employee's left index fingertip was pinched between the saw horse and the shim, resulting in a skin avulsion. The fingertip was amputated later.

Due to limited leverage and restricted positioning within the man basket of the crane, the employee exited the basket to manually adjust the waler from the structure itself. During the task, wave height and frequency increased. A large wave struck the area, causing the pile braces and walers to shift. The employee lost their footing and grabbed the production pile edge. Their fingers on one hand were crushed between two structural steel piles. The employee sustained an amputation to the fingers and was hospitalized.
