Puget Sound Naval Shipyard - Intermediate Maintenance Facility

On November 6, 2023, an employee was hydroblasting mold-in-place (MIP), a rubber hole coating, from the outer hole of a submarine when pressurized water lacerated the employee's left foot. The employee was hospitalized.

An employee was walking to the clinic when they tripped on a chain used to keep vehicles out of the ambulance loading dock. The employee hit their elbow on the sidewalk, sustaining an open fracture to the left humerus.

An employee was moving wood planks out of a machinery room on an aircraft carrier when they tripped and fell down 8 stairs of an inclined shipboard ladder, resulting in fractured ribs on their left side.

At about 11:30 a.m. on May 28, 2020, an employee was clearing a clog from the classification system for a plastic media recovery system. When the clog cleared, the open adjustable rotor started to spin, causing amputations to the employee's left index, middle, and ring fingers (with bone loss to the furthest knuckle on the middle finger).

An employee was using a portable worm-drive circular saw to cut slots in 4X4 lumber for trailer stairs and cut their left middle finger resulting in a fingertip amputation without bone loss.

After moving a barge lid, an employee was walking down the stairs on the barge when they slipped and fell approximately 15 to 20 feet, reuslting in a head injury.

An employee was using an excavator on a barge to scoop and remove fertilizer product stored in the cargo area. As the employee was transporting the skid-steer by chains and hoisting equipment from the dock to the barge, the employee fell approximately 10 feet off the skid-steer bucket to the cargo hold area below. The employee suffered a concussion.

Swingmen were performing discharge of an iron ore vessel. The discharge process consists of gathering remaining product into the center of the hold to facilitate removal by crane. At approximately 11:45 PM, a swingman was descending a ladder into a hold when he lost his footing and fell approximately 10-15 feet to the deck of the hold. The employee sustained fractures to their back and left leg.

An employee was on a bulk carrier ship. While removing lashing from wind turbine components, the employee lost balance and fell approximately 6 - 7 feet from a hatch to the deck of the ship. The employee was hospitalized with a head injury, and a fractured wrist, and required surgery.

An employee was rolling up a power cord inside the tank of a vessel when they stepped backward into a manhole behind them. The employee fell through the hole inside the tank of the vessel and sustained fractures to the shoulder as well as swelling of the left side of the back.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was struck by a plasma cutter and suffered a broken left tibia and fibula. The employee was hospitalized.

An employee was preparing to bend a flat bar in a brake press. When the machine was jogged, the stock rotated up and crushed his left middle fingertip against the outer frame of the die. The fingertip was amputated.

An employee was cutting metal with a torch. A piece of metal struck the employee's left foot, causing multiple fractures to metatarsal(s). The employee was hospitalized.

An employee was descending a ladder carrying a bag of trash. He fell, landed on the ground about 10 feet below, and suffered fractures to his right hip and pelvis.

An employee was drilling a hole into the sealed hold of a vessel so a marine chemist could test its atmosphere. Upon drilling the hole, a gas that was under pressure was released and caught fire at the drill motor, where the employee was holding it. The employee was burned.

An employee was performing restoration activities outside a 45-gallon condensation tank of a boiler system. As they were performing post-maintenance activities after repairing one of the two discharge pumps, one of the discharge valves would not open. The employee closed the intake pump to cool off the system and open the discharge valve. Two employees opened the inlet valve to return the tank to service. After they cracked open the inlet valve, a side wall of the tank blew out, causing the injured employee to suffer steam burns to half their body.

An employee was testing and troubleshooting the gang cylinder valve located in the gang area of the sawmill. The employee's fingertip was amputated by the gang cylinder valve.

An employee was unlocking containers on deck. The employee was de-lashing while standing on the edge of the lid. The lashing bar he was holding came off the cone, causing him to turn 90 degrees and strike a railing behind him. The employee sustained four fractured ribs.

An employee parked a side-by-side on the fixed internal ramp aboard a cargo ship. They exited the side-by-side and went to inspect the cargo below. The side-by-side rolled toward the employee and struck them as they were walking toward the edge of the ramp. The employee was knocked off the ramp and landed on the lower deck approximately 8 feet below. The employee was hospitalized with a fractured shoulder.
