On May 05, 2017, an employee was operating an electric pallet jack. He was traveling with the forks behind him and was standing on the left side of the platform on the pallet jack. When he approached a staged pallet of product, he turned his pallet jack to the left and made a full circle to get under the pallet of product. As he was positioning his forks under the pallets, he hit another pallet of product. His right ankle was pinned between his pallet jack and the pallet of product, peeling off the skin and requiring surgery.
