An employee was clearing a blockage on the exhaust system at start-up while standing on the cooling bath rotor. The fins on the cooling bath roller cut through the employee's steel-toed safety shoe and amputated the right pinky toe. The machine was not guarded at the time.

DGS Import, LLC

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

On May 9, 2025, an employee was using an air hose to clear debris from the courtyard area when she slipped and fell. The employee was hospitalized with closed fractures to her left fibula, tibia, and medial malleolus.

On April 24, 2025, an employee was opening a cylinder door on a hydraulic bridge to remove lumber when their left middle finger was caught between the door and the hydraulic bridge, resulting in an amputation without bone loss.

Mason's Forest Products

Two employees were using a plywood chop saw. The injured employee was holding one end of the saw with their right hand when the operator on the other end pressed the start button. The saw blade contacted the injured employee's right hand, resulting in multiple finger amputations when it amputated the hand above the thumb.

Barry-Wehmiller Papersystems International, Inc.

On January 17, 2025, at 12:15 p.m., an employee was moving 11-gauge steel sheets from a holding cart using a pendant-controlled, 1-ton jib hoist equipped with a plate clamp lifting attachment. With the plate clamp empty and open, the employee began raising the hoist. The clamp became caught on the bottom of the cart, tipping the cart in the direction of the employee. Several sheets of steel slid off the cart and struck the employee, resulting in several broken ribs, a broken left leg and ankle, a fractured kneecap, and a torn left ACL. The employee was hospitalized.

CPG Building Products

An employee was operating an air hoist to rotate a blending mill counter-clockwise. After he connected the hook to the mill, the hoist was raised and his hand was pinched between the hook block and the ring on the mill. The employee sustained a partial amputation of his right thumb.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
