A temporary employee was removing a part that she had just fabricated from the rolling machine. She stepped on the foot pedal while her hand was near the roller, which snagged her glove and pulled two fingertips into the roller. Her right middle and index fingertips were crushed.

An employee was using an acetylene torch to cut piping. A flash fire occurred and burned the employee's left hand, chest, face, and left ear. The employee was hospitalized.

On July 9, 2024, an employee was troubleshooting a relay on the low voltage inside a water source heat pump when he contacted an electrical component and was shocked. The employee sustained a burn to his left index fingertip.

An employee was descending an 8-foot step ladder from the fifth rung when he missed a step and fell to the ground. The employee sustained a tibia fracture.

Two employees were installing a 6-inch test T in a vertical 6-inch cast iron storm riser. The T was to be placed approximately 2 feet above floor level and required cutting and removing a section of the 6-inch storm riser. The two employees placed a riser approximately 5 feet above the section they intended to remove and utilized a 5-foot piece of 2" cast iron to support the riser clamp and ultimately the 6-inch cast iron pipe above the section being removed. They then cut and removed the 12-inch section of pipe. Employee 1 began installing the rubber no hub gasket on the cast iron riser coming through the floor while Employee 2 was installing the rubber no hub gasket on the cast iron riser coming down from the floor above. As Employee 2 was installing the rubber no hub gasket on the upper section of pipe the riser clamp slid off the 2-inch cast iron. This resulted in the upper section of pipe falling when the no hub band holding it in place failed to support the weight of the section of pipe. The upper section of pipe fell on Employee 2's right thumb smashing it between the two sections of pipe. The employee's thumb was fractured and lacerated requiring surgery to repair.

Three employees were installing a no-hub band over a 12-inch cast iron 90-degree elbow and a stub that went to a roof drain. The no-hub band broke during installation, and they descended in a lift to get a new one. The elbow fell down on one of the employees from about 20 feet above. It struck his head, which caused him to fall and hit his head on the toe board of the scissor lift. The employee sustained injuries to the head, neck/spine, and shoulder.

While working at a customer's residence, an employee was cutting tape with a box cutter to wrap insulation for a piece of pipe. The box cutter struck a hardened object within the wall causing it to bounce back and puncture the employee's right eye. The employee was hospitalized.
