An employee was manually adjusting the battery on a forklift in order to add water when the battery shifted unexpectedly due to the forklift's position on an uneven floor. The employee's finger became caught between the top of the battery and the forklift structure.

An employee was on a two-step ladder performing cleaning activities in the de-oiler machine. The employee fell back and his right arm was caught in the equipment, resulting in a right forearm fracture that required surgery.

An employee was changing an insert in the tool head of a CNC machine. The employee had stepped up approximately 28 inches onto the door track of the machine. When the employee went to step back down to the floor, they lost balance and fell backward onto a rack of metal parts. The employee sustained a compression fracture to the spine.

An employee was packaging steel coils on a turntable. As he walked across the turntable, he slipped and fell onto a coil. His left bicep was lacerated by the slit edge of a coil.

An employee was monitoring a lumber-stacking machine when they slipped and fell into an unbanded bundle of lumber. The outer row of lumber then fell on their left leg, fracturing their leg and ankle.

An employee was putting parts away in a warehouse. He tripped and fell into metal shelving, resulting in a broken arm.

An employee fell approximately 10 feet from an elevated platform to the concrete floor. The employee sustained broken ribs and a collapsed lung.

An employee was removing a walkie pallet jack from a pallet in the freezer. As he was dislodging the pallet jack forks that were stuck under the wood pallet, he lost his footing and was spun around landing on the concrete floor underneath the racking. The employee sustained a fractured left hip.

On June 5, 2024, an employee was mixing oils, powders, and other ingredients to make an antioxidant blend (a food ingredient) in a tank. A flash fire occurred and came out of the tank, burning the employee's chest and shoulder.

An employee was working on a valve replacement when their left middle finger became caught in the butterfly flapper of the valve, resulting in a fingertip amputation.

An employee was walking when they were struck by a forklift. The employee's leg was caught between the forklift and a storage rack, resulting in a leg injury that required hospitalization.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
