An employee was doing a cutback on a piece of lumber using a chop saw. The saw hit a knot, the piece of lumber buckled, then kicked back and broke the kickback guard. The board was then pulled back into the blade, which lacerated the employee's left index and middle fingers.

An employee was descending a rolling ladder when she fell from the last step and her head struck the floor, resulting in a fractured pelvis.

An employee was adjusting self-dumping steel hopper to get it to latch onto the tines of a forklift. He rocked the hopper back and forth to dump. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
