An employee was performing maintenance on an 8,600-ton shear press. The employee was standing on a metal plate approximately 3.5 feet long by 1.5 feet wide. The metal plate had a weld fail which caused the plate to break and fall into a pit. The employee fell partially into the pit and sustained a fracture to the right femur.
An employee was operating an aluminum extrusion stretcher. The extrusion pinched the employee's right thumb, causing an injury that required hospitalization.
