An employee had just placed some timbers on a pipe stack. As he was moving away, a 16-inch by 10-foot pipe that was being stacked rolled onto the timbers past the intended placement. This caused the timbers to catapult over the pipe. One of the two timbers struck the employee on the right side as it was coming down, knocking him to the ground. He suffered fractures to the right wrist, nose, face, and skull on the right side. He was hospitalized.

An employee rigged a crate so that it could be removed from a shipping container. The pallet jack could not access the crate in the back of the container so a choker was attached to the crate and a forklift attached to the choker to pull the crate into a position where the pallet jack could reach it. As the forklift operator was pulling out the crate with the choker and the employee was walking away, the crate came apart and a 4 x 4 portion of the skid struck the employee who rigged it in the back of the head below their hard hat, resulting in a laceration. The employee was knocked to the floor and sustained a fractured ninth rib on his left side.

Two employees were removing a pressure safety valve when chlorine gas was released. The injured employee suffered upper respiratory irritation due to the chlorine gas.

An employee was climbing up a fixed structural ladder when he lost his grip and fell approximately 6 feet to the ground, resulting in a fractured left hip.

An employee had just completed a job and was returning to the operations control room to sign out of the unit. The employee used his right shoulder to push open the door and exit the building. The door closed on his left middle finger, resulting in a fingertip amputation.

Employees were unbolting the head of a product purge bin utilizing a high-speed cut saw. As the crew was cutting the last of 15 bolts on the purge bin head, a flash fire occurred resulting in burns to four employees.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

S & J Construction, LLC

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

RMVK Enterprise, INC.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

HB Fleming, Inc.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.

Employees were securing the fuel line of an outboard motor in preparation for removing the motor from a small vessel located in the equipment yard. A gasoline-related fire occurred and one employee sustained burns to both hands and forearms.

A marine diver/construction worker was working underwater, using a hydraulic dredge to clear sediment away from the base of a dam. The dredge's suction nozzle began migrating toward him, and then pulled in his right hand and forearm. He sustained compartment syndrome in the hand and forearm.

Bechtel Energy Construction Services, Inc.

An employee was standing on a sheet of -inch by 4-foot by 8-foot plywood on top of a rebar mat, supervising his crew. As he was moving to alert the crew to an incoming overhead load, he stepped off the plywood and his left foot slipped through the rebar mat and landed on the rebar mat below. His left ankle was broken and dislocated.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
