An employee was evaluating metal floor grates in a stairway platform. The employee attempted to reset a metal floor panel in the stairway platform when the grate broke free. The employee's thumbs were crushed between the dislodged grate and a cement step. Both thumbs were amputated.
A pile rack was being lifted during bridge construction. A piece of the load slipped off the rack and struck an employee, causing fractures to the scapula and pelvis.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
