An employee was picking up cardboard debris from the floor below the cardboard recycle conveyor. One of the cardboard bales weighing approximately 1200 pounds fell from the conveyor and struck the employee, resulting in bilateral rib fractures, fractured vertebrae, and a laceration.

An employee was closing a sulfuric acid line when sulfuric acid was released into the atmosphere and sprayed on their face, forearms, hip, back, and chest resulting in chemical burns.

On January 12, 2024, an employee was splicing a new roll of paper into a preconditioner on a corrugated line when the paper broke and the their right hand was caught in a nip point, resulting in a burn to their hand and a fractured wrist.

On October 20, 2023, an employee was clearing a jam when their hand contacted the labeler machine's infeed rollers causing amputations to multiple fingers.

An employee was holding a single sheet down to gap the feed gate while the machine was running. When the roll grabbed the sheet, it pulled the employee's finger into the roll, resulting in amputation of the left little fingertip.

An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was walking behind the rewind coding machine while on his phone. He rested his left wrist on top of the slitter bar and his finger came in contact with the roller, resulting in amputation.

The injured employee was assisting the bander operator with dressing units with dunnage. A unit became stuck on the conveyor. The injured employee was pushing the unit on the conveyor when a conveyor roller pulled his shoe in-between the rollers. The employee sustained crushing injuries to their left foot/toes.

An employee was inspecting and adjusting a tray in a thermoforming machine when the the trigger switch was touched, causing the machine door to close on their right hand. The employee sustained crushing injuries to the hand.

An employee was adjusting the tracking of a conveyor belt when their left hand/arm were pulled into the conveyor. The employee sustained fractures.

An employee was working to retrieve debris from a powered conveyor when the rollers contacted his right thumb, resulting in a laceration.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
