An employee was driving a small forklift down an aisle. The forklift went out of control, and his left foot and arm were pinned between it and a rack. He suffered fractures to the humerus and the foot.

An employee was walking through an aisle to a pick location. A forklift was passing the employee when they clipped the racking, pushing the forklift in the direction of the employee. The outrigger from the forklift struck the employee's left foot, resulting in fractures to three of their toes.

An employee was backing their electric pallet jack out from underneath storage racks. While backing out, the employee's hand was caught between the rack and the backrest of the pallet jack, resulting in a fracture to the right hand and tendon injuries to the middle and ring fingers.

An employee was struck on the left foot by a pallet jack after dismounting it. The foot was broken.

An employee separated two pallets of product, wrapped them, and then began to back his pallet jack under them. The jack pinned his foot against the rack, breaking his femur.

The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

An employee was operating an electric pallet jack when it pinned his right ankle against some racking. The ankle was broken and he was hospitalized.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.
