An employee was working near a shelf containing sheet metal parts when a forklift truck on the other side pushed a sheet metal part off the shelf. The part fell on the employee, causing a facial laceration and fractured right femur.

An employee had staged a forklift and was preparing to release a latch from the attached bin. The latch did not allow the bin to tilt forward. The employee manually pushed the bin while holding the latch open. When the bin bounced back, the employee's left middle finger was crushed, resulting in partial amputation via surgery.

An employee was moving sheet metal onto a pallet and sustained a hernia.

An employee was depositing a piece of sheet of metal into the container of a forklift when the employee was struck by a second forklift. The back tire ran over the employee's left foot and broke it. The employee was hospitalized, requiring surgery.

The pallet struck the employee s ankles, causing her to trip and hit her head on either the forklift mast or the pallet. The employee sustained lacerations across the back of her head.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was holding a piece of rubber while it was being cut on a band saw. The saw blade amputated his right middle fingertip without bone loss.

An employee was observing expansion rods on a coil expander going into a coil to ensure copper stubs weren't being bent. His left hand was caught between the locking block and locking block clamp on the power press machine, resulting in partial amputation of his left little finger from the distal interphalangeal joint.
