An employee was working to free sheets of rubber that were stuck together in a machine. When the sheets popped free, a rubber sheet snapped back and pulled the employee's hands against a non-powered roller resulting in a non-displaced forearm fracture. The machine was not running at the time.

An employee was using a knife to cut a piece of rubber when the knife slipped and cut the employee's left forearm, resulting in a laceration that required hospitalization.

An employee was checking the function of newly installed actuators when they were struck by a pressurized release pipe. The employee suffered fractures to the orbital socket and shoulder, requiring hospitalization.

The employee was hospitalized with a fracture, and their right middle fingertip was amputated.

As they were shutting the door on the back of their box truck they were struck by an electric all-terrain vehicle that was being serviced, resulting in a fractured left ankle, fractured elbow with bone through the skin, and fractured pelvis.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
