An employee was responding to a service call at an apartment that had lost electricity. While using a screwdriver to check the outdoor breakers, the employee received an electric shock, resulting in third degree burns. The employee was hospitalized.

An employee was in the process of maintaining the swimming pool. A PVC pipe burst and the employee received chemical burns to the left side of the face, chest and arm. The employee was hospitalized.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

An employee was working on sub-floors and was transferring a tool when he slipped and struck a 2x8 floor joist. He fell into the sub-floor void on his right side. The employee was hospitalized with fractured ribs and a puncture to the liver.

An employee was in his car in the parking lot of his workplace (apartment complex) when a resident shot him with a gun. The employee was hospitalized with gunshot injuries to his right ribs and shoulder.

An employee was performing janitorial activities and unclogging a toilet and drain line that had shop rags and paper towels in it. After snaking the line to remove the obstruction, he was using a drain cleaning product and after about 20 minutes he became dizzy and lost consciousness as he was walking out of the room. He fell to the floor and sustained two fractured cervical vertebrae and bruising to the spine. The employee was hospitalized.

On February 14, 2025, an employee was walking when they slipped and fell, striking their head on the concrete ground. The employee was hospitalized.

An employee was using a table saw to cut a piece of wood. The saw cut his finger(s), causing an amputation.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
