An employee was using a coring/swaging circular saw and sustained severe lacerations to their left index finger requiring hospitalization.

An employee was removing straps from a load of 18" pipe in a trailer. The employee was pulling a release strap from the passenger side of the trailer when one unsecured pipe rolled from the top of the load and struck the employee's head and neck, causing a contusion.

An employee was unloading pipe from a drop-side trailer. He lowered the gate, but the pipe would not roll out. The employee went to the opposite side to push the pipe with his right arm. The pipe fell on and fractured his upper right arm, requiring hospitalization.

An employee was attempting to clear a product jam from a piece of processing equipment. When the product became free, the employee's hand struck a gear in the equipment resulting in a fracture to the index and little finger. The employee was hospitalized due to a surgical procedure that required the partial amputation of the little fingertip.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was sweeping the entrance of a building. Her right foot caught on the corner of a carpeted area, causing her to fall. She suffered a broken right hip and was hospitalized.

An employee was walking toward the custodial closet when they stepped on a corner of the corridor where there was some floor stripper on the floor. They slipped on the wet solution and struck their head on floor. The employee lost consciousness and was hospitalized.

An employee was trimming a tree with a chainsaw and lacerated their left ring and little fingers, leading to amputation and hospitalization.

An employee was using a snowblower to remove snow at a residential property and amputated two fingers.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
