The employee was making a y-turn maneuver to position the telehandler perpendicular to the sidewall and raise the small post frame truss up and over the 12-foot sidewall. The telehandler was on a steep grade. As the employee was making the maneuver, the telehandler began tipping to the side. The employee attempted to jump from the operator's seat and was struck by the roll cage. The employee was hospitalized with lacerations to the forehead and fingers.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

While installing diagonal bracing on the top chord, the trusses collapsed, causing the employee to fall approximately 20 feet to the concrete and be struck by the falling trusses. The employee was hospitalized with a fractured pelvis and a concussion.
