An employee tripped on a slope while carrying a box of supplies. Her leg buckled and she fell, the box landing on her leg. She suffered a leg fracture and was hospitalized.

The injured employee was working in a two-person team conducting troubleshooting and repair of an onboard boat engine. The engine started and the injured employee's right index fingertip was amputated. The cowling guards had been removed to provide access to the engine components.

An employee was drying and finishing vehicles when a vehicle door closed on their left middle fingertip and amputated it.

An employee was climbing down from a tractor trailer bed when they lost their footing and grabbed onto the roll-up door to stop the fall. The roll-up door then fell onto the employee's hand, resulting in a hand fracture that required surgery.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

An employee was mopping a floor when they stepped forward onto a section they just mopped and slipped, falling to the ground. The employee suffered a shattered kneecap.

An employee was walking through a swampy area carrying a machete. He tripped on a log, and the machete lacerated his right hand. He was hospitalized and required surgery.

An employee had just delivered a package at a local FedEx office and was walking back to his vehicle in the parking lot. Another vehicle backed out of a parking spot and pinned the employee between their vehicle and another parked vehicle. The employee's lower right leg was crushed, and they were hospitalized.

An employee slipped on an iced-over metal grate. The employee fell to the ground and suffered a broken left hip.

An employee was conducting a routine equipment check when a flash fire occurred and the employee sustained first-degree burns to their face, second-degree burns to the right side of their lower back and third-degree burns to the entirety of both legs.

An employee was on a 6-foot A-frame ladder, working to replace ceiling tiles. They fell to the ground and sustained fractures to their hip and femur. The employee was hospitalized and required surgery.

An employee was installing shoring and stepped on a 2x4 that was running across the top of a trench. He fell through and was hospitalized with a fractured left femur.

An employee was taking a break from powerwashing a deck when a door closed on his left index fingertip, resulting in an amputation above the nail.

An employee was securing a ramp to a dock to unload cargo. He was using large pins to put the ramp in place when two of his fingers got caught in the ramp, resulting in a partial amputation.

An employee was helping to dismantle a mobile crawler crane. Employees were separating boom sections. When the last pin was hammered out, the 40-foot boom section dropped 4-5 inches onto the injured employee's left foot. All five toes were amputated and the employee was hospitalized for surgery.
