An employee was standing on a step stool, preparing an encapsulation machine to be cleaned by putting a piece of plastic over the top part of it. As he was tucking in the plastic with his right hand, the machine's front access door closed on his right index finger. The right fingertip was amputated under the nail.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
