After finishing work on a fence, an employee started picking up tools to put them in a golf-cart. The employee was in front of the golf-cart when it struck her. She sustained several fractures including a broken leg and ankle that required surgical repair.

An employee was standing on the forks of a ride-on electric pallet jack which were raised approximately 9-12 inches from the floor to retrieve a box that was approximately six feet from ground level.
