Final narrative
An employee was standing on the forks of a ride-on electric pallet jack which were raised approximately 9-12 inches from the floor to retrieve a box that was approximately six feet from ground level. The employee's knee gave out and the employee fell to the ground hitting the forks of the pallet jack in the process. The employee sustained a possible pelvic fracture.
After finishing work on a fence, an employee started picking up tools to put them in a golf-cart. The employee was in front of the golf-cart when it struck her. She sustained several fractures including a broken leg and ankle that required surgical repair.
An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.
