An employee had just inspected a roof to find a leak. The employee fell from an upper roof to a lower one about 8 feet below and sustained fractures to their sternum and ribs. The employee was hospitalized.

An employee was in transit to take a picture of a scaffold tag when he was struck by a wood plank that came from an upper level. The employee was hospitalized with a laceration and fractures to their cheek, top palette, and orbital bone.

During roof repair, an employee was moving on the roof to establish a new tie-off point when they stepped on a roof panel that gave way and fell to a lower level. The employee sustained broken ribs.

An employee was setting up a ladder to access a roof. While tying off the ladder, the employee fell approximately 18 feet. The employee sustained injuries to his pelvis and right shoulder and required surgery.

An employee was pulling a four-wheeled cart out of a truck. The cart's wheels got caught on a ramp running from the loading dock to the truck; the cart flipped over and the employee fell backward, landing on the floor and suffering hip socket fractures.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

An employee was walking to the inventory cage when her ankle gave out and she fell to the floor. The employee sustained a femur fracture and was hospitalized.

An employee was dislodging a pallet that was stuck when boxes of product fell onto them, causing them to fall to the ground. The employee suffered a fractured femur.

An employee was stepping backward through a framed wall when they tripped over the base metal track and fell to the ground, resulting in a broken arm.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
