An employee was standing on a six-foot A-frame ladder to access a drop ceiling when he lost his balance and fell off the ladder, fracturing his ribs. He was hospitalized.

An employee was assisting in placing a slip sheet under an overturned pallet containing bread. When lifting the cases of bread, the employee sustained an annular tear to two lumbar vertebrae.

An employee was adjusting push-back rack rollers in a freezer to remove a jam. When he released three of the rollers, the fourth crushed his finger. The tip of his little finger was amputated.

Employee 1 was being trained on operating a powered industrial truck (PIT). The forks from Employee 1's PIT went into the operator's compartment of the trainer's (Employee 2) PIT and struck Employee 2's right heel, resulting in a calcaneus fracture.

An employee was retrieving two cargo airbags for loading a truck. While driving a powered industrial truck (PIT) down the dock, the PIT collided with a bollard by a door. The employee sustained a left thumb amputation.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was restocking a refrigerated area of a warehouse when he slipped on condensation and fell to the floor, resulting in a fractured hip.

An employee was operating a double pallet jack. While he was pushing a pallet back, the pallet jack moved and the handle crushed his finger against racking.

An employee was struck by a pallet that fell from an empty pallet storage bay. The employee sustained a dislocated shoulder.

An employee was offloading tools from a van and walking them over to the curb. He lost his footing and fell while carrying a jackhammer. The jackhammer fell and caught the employee's right little finger against the curb. The employee's finger was amputated.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

An employee was troubleshooting a power washer in the field. Because there might have been water in its fuel, he brought it back to the shop and drained about a gallon of fuel from the tank into a plastic container. Some of the fuel spilled onto the floor and ignited. The employee was stomping out the fire when he lost his balance and tripped into a stool, which caused the plastic container to spill more fuel onto the fire. The employee's pants and shirt caught on fire, and he fell, abrading his knee while trying to get through the flames. As well as the knee abrasion, he suffered burns to the left leg and left lower quadrant of the torso. He was hospitalized.

An employee was backing up a tram (towing powered industrial equipment) to connect it to a trash bin. The employee's left forearm/wrist was caught and crushed between the tram and the bin. The employee was hospitalized.

An employee went to lift a carton and tripped on a different carton that was on the floor. She fell on the floor in the backroom and sustained a fractured right hip, and abrasions to her arm and knee. The employee was hospitalized and required surgery.

An employee was conducting a delivery of beverage products and began having cramps due to dehydration. The employee was hospitalized.
