Federal OSHA recorded a severe workplace injury at Turner Industries Group, 900 Highway 108, SULPHUR, LOUISIANA 70665 on — Amputations, affecting the fingertip(s).

An employee was tightening bolts on a flange attached to a pipe. The employee was repositioning the pipe by moving the trunnion back and forth to allow access to the bolts on the flange. The trunnion moved towards the employee and he tried to grab the base plate to try and hold it up. The base plate struck the ground and caught the top of his left hand between the plate and the ground causing fingertip amputations to his left middle and ring fingers.

Turner Industries Group

An employee rigged a crate so that it could be removed from a shipping container. The pallet jack could not access the crate in the back of the container so a choker was attached to the crate and a forklift attached to the choker to pull the crate into a position where the pallet jack could reach it. As the forklift operator was pulling out the crate with the choker and the employee was walking away, the crate came apart and a 4 x 4 portion of the skid struck the employee who rigged it in the back of the head below their hard hat, resulting in a laceration. The employee was knocked to the floor and sustained a fractured ninth rib on his left side.

Two employees were removing a pressure safety valve when chlorine gas was released. The injured employee suffered upper respiratory irritation due to the chlorine gas.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.

Ahtna Marine and Construction Company, LLC

Employees were securing the fuel line of an outboard motor in preparation for removing the motor from a small vessel located in the equipment yard. A gasoline-related fire occurred and one employee sustained burns to both hands and forearms.

A marine diver/construction worker was working underwater, using a hydraulic dredge to clear sediment away from the base of a dam. The dredge's suction nozzle began migrating toward him, and then pulled in his right hand and forearm. He sustained compartment syndrome in the hand and forearm.

Bechtel Energy Construction Services, Inc.

An employee was standing on a sheet of -inch by 4-foot by 8-foot plywood on top of a rebar mat, supervising his crew. As he was moving to alert the crew to an incoming overhead load, he stepped off the plywood and his left foot slipped through the rebar mat and landed on the rebar mat below. His left ankle was broken and dislocated.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

CHRISTUS St. Frances Cabrini Hospital

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

Jean Simpson Personnel Services, Inc.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
