An employee was operating a boom in a bucket truck. One outrigger of the truck was placed on the pavement and the other outrigger of the truck was on soft ground. The outrigger that was on soft ground sunk into the ground and caused the truck to roll and the weight to shift on the truck. The employee and bucket then fell to the ground. The employee suffered L1 and L2 vertebrae fractures and a right cheek laceration. Fall protection was worn at the time of the incident.

An employee was manually pruning a tree when he fell approximately 30 feet to the ground below, sustaining multiple lacerations, broken ribs, fibula/tibia fractures, pressure buildup, a cheekbone fracture, a skull fracture, and two fractured vertebrae. He was hospitalized.

The scissor lift guard rails were below the girder of the crane to avoid contact. The tow arm for the crane collectors contacted the guardrail of the scissor lift and caused it to tip over. The employee stayed inside the scissor lift basket as he fell to the floor. The employee sustained fractures to the right arm and hip.

An employee was on a forklift that was elevated approximately 20 feet and was auditing inventory. The forklift malfunctioned and the employee fell down to the surface below, causing them to sustain multiple fractures.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
