Final narrative
An employee was cleaning/servicing a powder-actuated tool. While the tool was disassembled and the piston was against the palm of his left hand, the trigger was actuated. The tool was loaded with boosters and projected the piston into the employee's left hand/wrist. The employee sustained a punctured left hand.
An employee was operating a forklift while holding the spotters line. The slack from the line wrapped around the back wheel of the forklift and when the wheel moved it tightened and crushed the employee's left hand.
An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.
An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was on a 6-foot ladder, working to nail a 2-by-4 on a hotel carport ceiling. He lost his balance and his muddy boots slipped. He then fell and his foot lodged in the ladder. His foot was broken in two places and he was hospitalized.

While installing diagonal bracing on the top chord, the trusses collapsed, causing the employee to fall approximately 20 feet to the concrete and be struck by the falling trusses. The employee was hospitalized with a fractured pelvis and a concussion.
