A toolmaker was setting a pin in a die set using a hand-operated press with an overhead pull-down bar when the die set slipped. This released the resistance against the downward force causing the toolmaker to fall backward against a storage rack and then to the floor. The toolmaker suffered a head injury.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
