Crew members where unbolting a 40-foot riser pipe in order to separate the pipe in two pieces. When the bolts where removed, the pipes fell apart. The cable used to secure the bottom pipe caught an employee's left hand and pinched it between the pipe and the cable. causing a laceration to the middle finger and injury to the index finger. The employee was hospitalized.
An employee was leaving a scaffold to get onto a ladder. The tank sheet buckled, causing both the ladder and the anchor of the employee's lanyard to slide off. The employee fell approximately 16 feet and sustained a fractured pelvis.
