Federal OSHA recorded a severe workplace injury at Fruit of the Earth, 3325 West Trinity Blvd., GRAND PRAIRIE, TEXAS 75050 on — Fractures, affecting the foot (feet) and ankle(s).

An employee was using a 10-foot rolling stair ladder to access the top of a salt hopper. The employee fell to the ground 10 feet below, breaking both heels and his left ankle.

An employee opened an access door to adjust the tension on a belt for a bag sealer machine. During that task, the access door strut failed and he attempted to tighten the door strut bolt with a wrench. The employee's hand slipped from the wrench and the tip of his right ring finger came into contact with the belt gears located just below the door strut, resulting in an open fracture and amputation.

An employee was standing next to a building post when a tote, carried on a forklift, crushed his right ring finger. The tip was amputated.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

American Residential Products, Inc.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

Universal Beauty Products Incorporated

A temporary employee was cleaning the outside of a cylinder on a cream filling machine. The machine had been turned on so that leftover product could drain from it. The paper towel the employee was using fell into the cylinder, and as the employee went to retrieve it, the blade inside the cylinder amputated the tip of his right middle finger.

An employee was operating a ride-on powered pallet jack when his left calf was pinned between the pallet jack and a bollard. He was hospitalized with a laceration to the calf.

An employee was capping bottles of skin therapy oil when the plastic cover for the light fixture fell and struck them on the head. The employee was hospitalized with a head contusion.

An employee was troubleshooting a capper machine that places caps onto cosmetic containers. His left hand was pulled into the machine, resulting in an injury to his middle finger and a ring finger amputation.
