An employee was on a 6-foot ladder conducting maintenance on a ballast above the ceiling in a hallway. When the employee removed the ballast he was shocked with 208V of electricity and fell off the ladder, landing on his left shoulder. The employee suffered an electric shock and a dislocated shoulder.

An employee was on a ladder replacing lights on an ATC tower. He slipped off a rung of the ladder and fell to the ground, sustaining broken ribs.

An employee was walking from the terminal to the rental car area when they tripped and fell on the ground, sustaining a fractured knee. She was hospitalized and required surgery.

An employee was descending a fixed ladder on the back of an antenna inside a radome. He fell from the ladder to the radome deck about 12 feet below. He suffered a broken heel and a broken rib.

An employee was investigating a noise in an HVAC duct-system. He located a suspect auxiliary blower unit, deenergized it, and then went to retrieve tools. He returned to the blower after approximately 10 minutes. The fan/belt/pulley were slowly rotating even though power had been isolated. The employee removed a cover so he could press a tool against the belt to bring the blower to a stop. While reaching for the tool, his hand slipped into the pulley/belt resulting in an amputation to the right index finger from the base of the nail to the end of the finger. Also, the top third of the right middle finger was degloved.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee turned off a pushback tractor at a gate, put it in neutral, and put the emergency brakes up. The employee then exited the tractor and noticed it was rolling forward toward the aircraft. The right side of his body was wedged between the tractor and the aircraft nose. The employee was hospitalized for fractured ribs and a partially collapsed lung.
