An employee was cleaning the box chute for a crusher when his left hand slipped between the rollers while they were still turning, causing trauma to the hand and forearm and an amputation.

An employee was cleaning debris from the rollers of a calendar machine while it was in operation. The mill cleaning cloth the employee was using caught on one of the rollers and the employee was pulled in between two rollers. He sustained crushing injuries to the upper body and head.

An employee was carrying an extension receptacle outlet (240 V) up a flight of process equipment stairs. The receptacle contacted the stair handrail at which time the employee experienced an electrical shock for an extended period of time.

An employee was cleaning a drum roller and material belt when his hand was caught between the belt and drum roller, which then pulled his arm into the belt and roller. He suffered a broken wrist; three broken ribs; burns to the forearm, palm, and fingers; and skin removal from the forearm, palm and fingers.

An employee was driving a golf cart toward the golf cart parking area. The employee was driving too fast and struck the injured employee, pinning him between the wall and golf cart. The injured employee broke his left tibia and knee.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was cutting a 7-foot pipe using a band saw. He went to wipe moisture off the pipe with a rag and the rag became entangled in the moving saw blade, pulling his right hand into the blade. The employee sustained a partial amputation to the fourth distal phalanx and third digit.

An employee was working in the grinding room on a production line for bathtubs. The tubs are laid on a conveyor and have wooden blocks for feet. The conveyor moves the tubs onto a saw that cuts off the wooden blocks. The saws are protected by expandable metal guards. The conveyor then moves the tubs through an area lined with coarse horsehair brushes before reaching the discharge area, where the employee was working. The employee was drilling a hole behind the metal guarding when their right hand went into the horsehair brush area and contacted a saw blade, resulting in a laceration on the back of their right hand that required surgery.

An employee was taking a part out of a press. The press's injector pin came down on his right big toe, crushing it. He was hospitalized.

An employee was cleaning saw dust off of a straight line rip saw after turning it off when his fingers contacted the blade that was still spinning, resulting in the amputations of the left middle and index fingertips to the first knuckle.

An employee was fusing two HDPE pipes in a machine when the machine amputated his left index and middle fingertips.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
