A turret forklift operator was reaching for a two-way radio when his finger contacted an energized (85V DC) wire on the forklift. The employee sustained an electrical shock and burn on his finger and was hospitalized.

The pallet struck the pallet above it, causing the pallet on the third level to fall. The employee was struck by the falling pallet of 30-pound cardboard boxes filled with poultry products and he sustained fractures to his right tibia and fibula that required surgery.

Two employees were performing preventative maintenance on a machine. The injured employee noticed excess grease on the drive system and wiped it off with a rag while the machine was turned on and running at a slow speed. The rag was caught and the employee's hand was pulled into the drive system resulting in a partial amputation of his right index finger.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.
