A turret forklift operator was reaching for a two-way radio when his finger contacted an energized (85V DC) wire on the forklift. The employee sustained an electrical shock and burn on his finger and was hospitalized.

An employee was stacking cases of product, each weighing approximately 10 - 20 pounds, onto a pallet inside a railcar. A pallet that was stored on an upper level dislodged and fell, striking the employee from behind. The employee sustained a dislocated left hip.

On November 22, 2024, an employee was pulling a pallet of product from a second-level location using a high-reach lift. The pallet struck the pallet above it, causing the pallet on the third level to fall. The employee was struck by the falling pallet of 30-pound cardboard boxes filled with poultry products and he sustained fractures to his right tibia and fibula that required surgery.

Two employees were performing preventative maintenance on a machine. The injured employee noticed excess grease on the drive system and wiped it off with a rag while the machine was turned on and running at a slow speed. The rag was caught and the employee's hand was pulled into the drive system resulting in a partial amputation of his right index finger.

On April 25, 2024, at about 5:15 PM, an employee was receiving a trailer load of product to be unloaded at a dock. The employee became pinned between the reversing trailer and the dock bumper, resulting in a contusion to the lower back and torso.

An employee and their team were fixing a floor tile in the meat department of a supermarket. The employee went to move a large meat grinder when he was electrocuted due to a loose 220-volt ground wire. The employee was hospitalized with an electrical shock to the right shoulder that spread throughout his body.

An employee was servicing a pool heater and was electrocuted when their right elbow came into contact with a 220-volt electrical line.

An employee was changing the saw band and guide blocks on the resaw at the end of a shift. As the employee reached to change the guide block on the right side, they contacted the power cable for the photo eye and sustained a 110-volt shock from an exposed wire. The employee was shocked under their arm and on the right side of the body and sustained a burn.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
