An employee was using an electric pallet jack to unload a trailer. While a pallet of product was being moved, it struck the trailer's overhead door and fell onto the employee. The employee suffered a severe head injury and was hospitalized.

A stack of items on a pallet jack was being moved through a trailer door. The top of the stack struck the door causing the door to fall onto an employee. The employee suffered broken ribs and torso contusions.

An employee was opening a roll-up door when his glove was caught in the door's rope, resulting in a partial amputation to his right index fingertip.

An employee was pulling products from a storage rack when a damaged cardboard box collapsed, causing a pallet full of product to fall approximately 15 feet. The 700-pound pallet struck the left side of the employee's body, resulting in an injury to the left shoulder and hip.

An employee was backing up a stand-on pallet jack. The employee's right foot and toes were caught and injured between the pallet jack and a parked one.

An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.

An employee went to step off material handling equipment to grab their jacket when the throttle of the vehicle engaged. The vehicle reversed and the employee's left foot was crushed between two material handlers, resulting in a fracture.

An employee was working at a height of approximately 20 feet and was standing outside his forklift's basket attachment. His forklift was struck by another forklift, causing the employee to fall to the warehouse floor below. The employee sustained facial injuries, broke two ribs and an arm, and also suffered a head injury. The employee was wearing a harness at the time but it was not connected to an anchor point.

On November 8, 2023, an employee was operating a stand-up pallet jack in the warehouse. As he was backing up, the pallet jack struck the basket of a parked aerial lift. His right ankle became caught between the pallet jack and the aerial lift basket resulting in a fractured ankle.

On October 26, 2023, at approximately 12:00 PM, an employee operating a four wheeler was on their way to an off-property break location. The employee stopped the four wheeler near the administrative office to allow a log truck to exit the property. The log truck backed up, and the employee's left ankle was caught between the log truck trailer and the four wheeler, resulting in a fracture and laceration.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

An employee was operating an electric pallet jack when it pinned his right ankle against some racking. The ankle was broken and he was hospitalized.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
