On December 23, 2020, an employee was cleaning overhead piping using an extended brush on an 8-foot A-frame ladder. The ladder began to tip and the employee fell to the floor, suffering two broken bones in the ankle that required surgery.
On August 24, 2019, an employee was unplugging the rotary valve under a cyclone when the employee's left index finger contacted the moving rotary valve, resulting in a partial amputation of the left index fingertip.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
