On January 4, 2021, an employee was operating a pneumatic press with an operating pressure of 400-700 psi. The employee as cycling the press when they reached down to retrieve a cutting die that fell off the staging block. The employee's left index finger was caught in the point of operation, resulting in an amputation.
On February 11, 2016, an employee was performing a flare test on a 5" metal tube. The employee was holding the tube on the test cone and cycled the hydraulic press. The tube slipped off of the test cone, pinching the employee's finger between the tube and the bottom of the press. The employee's left pointer fingertip was amputated.
